Dear Parents,
The district is in the process of switching to a new student database management system. Our current system has served the district well for over twenty years. We will be changing to this new system to allow us to complete required state and federal reports in addition to other features that will allow us to automate other procedures in the district over the next few years. For example, one of the added benefits will be the automation of the school libraries during the 2008-2009 school year.
The adoption of the new database necessitates the revision of all of our registration forms. With that in mind, it will be necessary for all parents to establish residency and complete new registration forms for all students. Please read the instructions and forms carefully as some of our procedures have changed.
The book/materials fee for the 2008-2009 school year is $60.00 per student. A reduced fee of $50.00 per student will be allowed if full payment is received during the registration times on August 12, 13, or 14. The discounted fee is offered because we are better prepared to process all necessary paperwork with added personnel during the registration dates prior to the start of school. To assist you in planning for registration please be aware that junior high gym suits cost $22.00 and the planner costs $7.00. Our hope is to encourage registration prior to the start of the school year so that the entire focus of the first week of school will be on students and learning.
Once again, Brookwood School District 167 will conduct centralized registration sessions in order to allow you to register all of your children at one site. Registration will be held at Brookwood Junior High School for the entire school district.

Registration forms for the 2008-2009 school year are included with this letter.
1. Form A – Proof of Residency
Form A must be completed by the parent/guardian enrolling a new or returning student in the district for the 2008-2009 school year. Only one form per family is required. Please review this form carefully and be prepared to show documentation that proves your child’s residency in the district. The document that you use from Category 1 must display your current address in the district. In order to use an affidavit to prove residency, the parent/guardian of the child and the owner of the residence must be present together during registration and complete Forms F and G. The owner of the residence must establish residency with the required documentation listed in Form A.
2. Form B – Registration Form
Form B must be completed for each student enrolled in the district for the 2008-2009 school year. Please make sure that both sides are completed accurately.
3. Form C – Emergency Form
Form C must be completed for each student enrolled in the district for the 2008-2009 school year. Please make sure that both sides are completed accurately as this information will be used in the event of an emergency.
4. Form D – Home Language Survey
This form is available in English and Spanish and must be completed for each student enrolled in the district for the 2008-2009 school year. This form is required per the State of Illinois School Code.
5. Form E – Letter to Household Regarding Free/Reduced Lunch
Completion of Form E is optional. Only one form per family is required. Please review the directions for this form to determine your child’s eligibility for free/reduced lunch benefits. A Direct Certification Letter for Free Lunch from the State of Illinois may be used in lieu of Form E.
6. Form F – Acknowledgement of Responsibility
Form F is to be completed by the Parent/Guardian of the child and the owner of the residence. Form F is completed in conjunction with Form G. Please keep in mind that Public Act 89-480 specifies penalties for fraudulent registration of a student as a Class C misdemeanor with potential penalties of up to 30 days in county jail or a $1500.00 fine or one year of probation for the parent/guardian and owner of the home.
7. Form G – Affidavit of Residence
Form G, an Affidavit of Residence should only be completed by those parents/guardians who are not able to produce the required documentation to establish residency in the district. This form must be signed by the parent/guardian and homeowner and notarized. Form G is completed in conjunction with Form F. Please keep in mind that Public Act 89-480 specifies penalties for fraudulent registration of a student as a Class C misdemeanor with potential penalties of up to 30 days in county jail or a $1500.00 fine or one year of probation for the parent/guardian and owner of the home.
A waiver of fee form will be available if your family qualifies for free lunch. This form applies only for free lunch not reduced lunch. Documentation such as a food stamp verification letter, Temporary Assistance for Needy Families (TANF) verification letter, or income verification such as a letter or current check stub is required for approval of the waiver of district fees.
If you are planning to move or transfer your child(ren) from Brookwood School District 167, please report to your child(ren)’s school to fill out the appropriate transfer forms.
