Form A must be completed by the parent/guardian
enrolling a new or returning student in the district for the
2009-2010 school year. Only one form per family is required. Please
review this form carefully and be prepared to show documentation
that proves your child’s residency in the district. The document
that you use from Category 1 must display your current address in
the district.
In order to use an affidavit to prove residency, the
parent/guardian of the child AND the owner of the residence must be
present together during registration. Additionally, the owner of
the residence must establish residency with the required
documentation listed in Form A.
Form B must be completed for each student enrolled in the district for the 2009-2010 school year. Please make sure that both sides are completed accurately.
Form C - Non-Custodial Parent Mailing
Information
This is an optional form to be filled out by a
parent or guardian who does not reside with the student and would
like to
receive mailings from the school.
Form D must be completed for each student enrolled in the district for the 2009-2010 school year. Please make sure that both sides are completed accurately as this information will be used in the event of an emergency.
This form is available in English and Spanish and must be completed for each student enrolled in the district for the 2009-2010 school year. This form is required per the State of Illinois School Code.
Form F -
Letter to Household Regarding Free/Reduced Lunch

Completion of Form F is optional. Only one form per family is required. Please review the directions for this form to determine your child’s eligibility for free/reduced lunch benefits. A Direct Certification Letter for Free Lunch from the State of Illinois may be used in lieu of Form F.
Form G - Acknowledgement of Responsibility
Form G should only be completed by those parents/guardians who are not able to produce the required documentation to establish residency in the district. Form G is to be completed by the Parent/Guardian of the child and the owner of the residence. Form G is completed in conjunction with Form H. Please keep in mind that Public Act 89-480 specifies penalties for fraudulent registration of a student as a Class C misdemeanor with potential penalties of up to 30 days in county jail or a $1500.00 file or one year of probation for the parent/guardian and owner of the home.
Form H - Affidavit of Residence
Form H, an Affidavit of Residence, should only be completed by those parents/guardians who are not able to produce the required documentation to establish residency in the district. This form must be signed by the parent/guardian and homeowner and notarized. Form H is completed in conjunction with Form G. Please keep in mind that Public Act 89-480 specifies penalties for fraudulent registration of a student as a Class C misdemeanor with potential penalties of up to 30 days in county jail or a $1500.00 fine or one year of probation for the parent/guardian and owner of the home.
