The Following Items Are Required To Prove Residency:

• A copy of the birth certificate if your child has not previously attended a school in District 167

• Proof of vision exam for kindergarten students or students new to Illinois

• Four proofs of residency which must include the name and current address of the parent/legal guardian. One of these proofs of residency must be in the form of a picture ID and current lease or mortgage. You will not be able to complete the registration process if your name and current address are not displayed on the documents you are using to prove residency

• Student Transfer Form in good standing if transferring from another school

• Proof of dental exam for students in Grades K, 2, and 6^th

• Proof of  physical exam with required immunizations for students in Pre-K, K, & 6^th grade. Health forms can be downloaded from the district website, please click on this link “Registration Forms”.

The window for [Online Registration Enrollment] for the upcoming 2019-2020 school year will be open from [July 8^th until July 19^th]. Parents of returning students will receive a letter in the mail during the week of July 1^st with directions and a [Snap Code] that you will use to register online (this letter serves as proof of residency). You can log on immediately with your snap code and begin registering from any computer with an Internet connection or by phone.

You may also visit your local library to access a computer.

If you do not receive a Snap Code in the mail or if your child is [new to the district], you will have to register in person at your child’s school during the week of August 12^th, 2019 and prove residency. August 12^th, 12:00 p.m. – 7:00 p.m., August 13^th – August 16^th, 9:00 a.m. – 3:30 p.m. Last registrant will be taken 30 minutes before the end time.

Pre-K & Kindergarten registration will be held on August 7^th from 8:00 a.m. – 2:00 p.m. and on August 8^th from 12:00 p.m. – 8:00 p.m. at your child’s school. The last registrant will be taken 30 minutes before the end time.

• A Technology Fee of $100.00 (per family) is due at time of registration for all students unless you qualify for a waiver.

• An additional $50.00 Late Fee (waiver does not apply) will be applied to returning students that do not register online during online enrollment from July 8^th, 2019 – July 19^th, 2019.
