Driver's License or State l.D. From The Homeowner And From The Parent/Legal Guardian
Current Tax Bill, Current Mortgage Documents, or Signed Current Lease (Must be Verifiable with the Village of Glenwood) From The Homeowner
Two (2) Current Documents from the List Below (within the last 30 days) that has the District Address from both homeowner and the parent/legal guardian

Proof of Vision Exam for Kindergarten or Students "NEW" to Illinois
Proof of Dental Exam for Students in Grades K | 2^nd | 6^th Grade
Proof of Physical Exam with Required Immunizations for Students in Pre-K | K | 6th Grade
Student Transfer Form In Good Standing if Transferring from another School
